About the Transfer Your Care Program
Yes. Revibe Health welcomes patients transferring their care from other clinics or programs, including local clinics, medical practices, and online or telehealth companies. Contact us for a medical release form — our clinic staff will coordinate the entire transfer to your nearest Revibe Health location.
Transfer Your Care is the process for moving your existing treatment to Revibe Health without starting from zero. You share the records you already have, a licensed Revibe Health provider reviews them, and the two of you build your plan together at your first visit. It is not a separate treatment program — it is how you become a Revibe Health patient when you are already in treatment elsewhere.
You can transfer care for the treatments Revibe Health provides: hormone therapy for men and women, medical weight loss, sexual health, and hair restoration. If you are unsure whether your current treatment is something Revibe Health offers, call your nearest clinic and the care team will tell you before you book.
Yes. Many Revibe Health transfer patients started with an online or telehealth provider. Bring whatever your online provider gave you — lab results, a treatment summary, or the details of what you were prescribed and when. A Revibe Health provider reviews it before recommending your plan.
Yes. If you are already a Revibe Health patient and you move, travel, or simply prefer a different location, your care moves with you inside the Revibe Health network at no charge. Call either clinic and the team will coordinate it.
No. The Revibe Health consultation and exam is normally $199, and that fee is waived for transfer care patients. There is no charge to move your care to Revibe Health, and no charge to move between Revibe Health clinics once you are a patient.
Booking Your Transfer Appointment
No. You can book without them. Gathering records takes time and you should not wait on it. Book the appointment first, then bring whatever you have collected by the time you are seen.
There are no cancellation or rescheduling fees. Call your local Revibe Health clinic or send a message through the website SMS chat tool, ideally at least 24 hours before your appointment, and the team will find you another time.
Records, Labs and What to Bring
Bring a photo ID and whatever you have about the treatment you are already on. That means lab results from within the last six months, a photo of your current prescription label, and a note of the doses you have been on and when those changed. Bring what you have — you do not need all of it to be seen.
Bring lab results from within the last six months. Older results are still worth bringing — they show your provider how your levels have moved over time — but the six-month window is what counts as current. If your most recent labs fall outside it, book anyway and bring what you have.
Contact your nearest Revibe Health clinic and ask for a medical release form. Once you sign it, the Revibe Health clinic staff coordinate the request with your previous provider. You can still book and attend your appointment while records are being collected.
Yes. After you complete a medical release form, the Revibe Health clinic team coordinates the records request with your current or previous provider. You do not have to manage the paperwork yourself.
Your First Visit as a Transfer Patient
You will see a licensed Revibe Health healthcare provider. Across its clinics Revibe Health employs M.D.s, D.O.s, N.D.s, P.A.s, N.P.s, PA-Cs and MSNs. You can see who practises at your clinic on that clinic’s page before you book.
No. Revibe Health is not a pharmacy and cannot fill prescriptions written by another provider. Any medication you receive as a Revibe Health patient is prescribed by a licensed Revibe Health provider and filled through Revibe Health’s pharmacy partner. Bring your current prescription details so your provider can see what you have been taking.
A patient care coordinator or clinic manager greets you and takes you to a private treatment room. Visits are discreet and efficient. If you have questions before you come in, call the clinic directly.
Yes. Revibe Health emails you links to complete your health history and patient information forms online before your visit, and offers a patient portal for new patient intake paperwork. Completing it in advance means more of your appointment is spent with your provider.

A Revibe Health consultation and exam is $199, and that fee is waived for transfer care patients. Your consultation includes a meeting with a licensed provider, a medical health screening, and a personalized treatment recommendation.
No. Revibe Health does not accept insurance or Medicare. Pricing is all-inclusive and transparent, with no deductibles, separate lab fees or co-pays.
Revibe Health accepts cash, checks, all major credit cards, and HSA and FSA payments.
